Today we discussed the importance of documenting any condition or injury that a child arrives to daycare with and speaking with the parent immediately upon discovery to ensure an accurate account of when the condition or injury occurred is specifically documented and established.

An unannounced complaint inspection was conducted on September 8, 2017 in response to a complaint that was received in the licensing office on 11/18/14 regarding alleged noncompliance in the areas of Physical Plant , Staffing and Supervision, Programs and Special Care Provisions. Based on the information gathered, there was not sufficient evidence to support the allegations, so complaint is found to be not valid. However, a non-compliance which was not directly part of the stated complaint but was discovered in the course of investigating the complaint is identified on the violation notice. Description: Based on observation, the licnesee did not enure that all of the requirements for administering diaper ointment were consistently met. Evidence: 1. There were three tubes of diaper ointment on site during the inspection today. 2. A written parent authorization noting any known adverse reactions for Ointment #1was not available. The program director was unable to locate it during the inspection, but stated that she recalls one being submitted by the parent. 3. Ointment #3 did was not labeled with the child's name. The director and staff were unsure who the ointment belonged to. 4. A record was not available for any of the 3 ointments that includes the child's name, date of use, frequency of application and any adverse reactions. The program director stated that she was not aware that administrering of ointment needed to be documented.

Plan of Correction: A new authorization form for ointment #1 will be obtained from the parent today. Ointment #3 will be properly disposed of. A form will be created to document ointment use for each child who is administered ointment, including the child's name, date of use, frequency of application and any adverse reactions.
