22VAC40-185 STAFFING AND SUPERVISION.
An unannounced inspection was conducted to investigate a self-reported incident regarding supervision on July 25, 2017 from 2:15 p.m. to 3:15 p.m. Documentation was reviewed. Standard #: 22VAC40-185-240-B Description: Based on interviews and review of documentation, the facility failed to have a required policy and procedure for identifying where attending where all children are at all times. Evidence: The facility did not have procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center. Plan of Correction: Written policies and procedures are now available for all staff. These policies outline FCCC procedures for identifying where all children are at all times.
Standard #: 22VAC40-185-340-F Description: Based on interviews, not all children under the age for ten years old were within sight and sound supervision of staff at all times. Evidence: On July 12, 2017 at approximately 11:30 a.m., an eight year old child was left at the Farmer's Market during a field trip. The child was alone for approximately thirty minutes. Plan of Correction: The direct care staff responsible for the child was terminated. The Site Manager received disciplinary action. Training was given to all staff regarding supervision of children.
