An unannounced Subsidy Health and Safety Inspection was conducted on September 7, 2017 from approximately 8:30 a.m. until 1:05 p.m. Upon entrance there were 8 children and 4 staff present with children. The record review sample consisted of 5 children's records and 5 staff records. Children were observed eating snack, jumping singing and dancing. Standard #: 22VAC40-185-240-C Description: Based on record review and staff interviews, the licensee did not ensure program directors and staff who work directly with children obtained annually 16 hours of training. Evidence: On September 7, 2017, 5 records were reviewed and the record for staff #1 only included documentation of 10 hours of annual training and the record for staff #2 only included 14 and 1/2 hours of annual training. The records for staff #1 and staff #2 did not include the required 16 hours of annual training. Plan of Correction: The administrator will have staff get training.
Standard #: 22VAC40-185-410-2 Description: Based on observation and staff interviews, the licensee did not ensure children were not placed in a small confined space or any space that the child cannot freely exit himself; however, this does not apply to the use of equipment such as cribs, play yards, high chairs, and safety gates when used with children preschool age or younger for their intended purpose. Evidence: On September 7, 2017, 1 child, child #5, age 11 months was observed sitting in a highchair and center staff stated the chid was in the highchair to keep them from going into the bathroom and other places. Chid #5 was observed in the high chair from 8:38 a.m. until 8:59 a.m. Plan of Correction: The center administrator will discuss it and the high chairs will not be used for their unintended purposes.
Standard #: 22VAC40-185-570-E Description: Based on observation and staff interviews, the licensee did not ensure that prepared infant formula was refrigerated, dated and labeled with the child's name. Evidence: On September 7, 2017, there were 4 infants observed in care and 1 bottle belonging to one of the infants was not dated or labeled with the infants name. Plan of Correction: The administrator had a label put on it during the inspection.
Standard #: 22VAC40-185-570-I Description: Based on observation and staff interviews, the licensee did not ensure there was one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care maintained at the center. Evidence: On September 7, 2017, the center director confirmed there was not emergency formula or bottles available for the children in care. Plan of Correction: The administrator will purchase bottles, nipples and emergency supplies and request formula from parents.
