An unannounced monitoring inspection was conducted on September 12, 2017. At the time of entrance there were 29 children present with 5 staff members. The sample size consisted of 5 children's records. Children were observed eating breakfast, brushing their teeth, playing with blocks and during story time. There are no medications on site. The inspection began at approximately 8:50 a.m. and ended at approximately 11:15 a.m. Areas of noncompliance are noted on the violation notice. Standard #: 22VAC40-185-60-A Description: Based on a record review and staff interviews, the licensee did not ensure a separate record for each child was kept at the center and included all required information for each child enrolled. Evidence: During the inspection on September 12, 2017, 5 children's records were reviewed the following records were determined to be incomplete; 1) The record for child #1 did not include the fathers employment phone number and location, and previous childcare. 2) The record for child #2 did not include the fathers employment location and previous childcare. 3) The record for child #3 did not include proof of identify and previous childcare. 4) The record for child #5 did not include start date, proof of identity, the home phone number for mother and father, mothers employment phone number, fathers employment phone number and employment location. Plan of Correction: The center supervisor will get what is needed and have parents fill out the forms.
Standard #: 22VAC40-185-90--A Description: Based on a record review and staff interviews, the licensee did not ensure a separate record for each child was kept at the center and included all required information for each child enrolled. Evidence: During the inspection on September 12, 2017, 3 of the 5 children's records (child #3, #4, and #5) did not include the three parental agreements listed below; 1) An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds;The record for child #1 2)A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; 3)A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Plan of Correction: The center supervisor will get what is needed and have parents fill out the forms.
