Comments:
An unannounced monitoring inspection was conducted on 9/14/17 at 9:30 a. m. and concluded at 1:00 p. m. The inspector met with the assistant director and Child Day Center standards were reviewed.

The following staff-to-child ratios and activities were observed.

Infants: There were seven infants with two staff. The infants were observed having free play on the floor dancing to music.

Toddlers: There were five children with one staff. The children were sitting at the table preparing to do a food project with bananas.

Two Years: There were 14 children with two staff. The children were observed having circle time, the children and staff were sitting on the rug and one staff was reading the children a story.

Preschool (Three Years): There were nine children with one staff. The children were sitting at the table playing a board game "All about Me" with direction from staff.

Pre-K (Three-Four Years): There were nine children with one staff. The children were observed having story time.

Lunch consisted of chicken tacos, corn, pears and milk.

Eight staff records were reviewed.
Eight children's records were reviewed.
No medications at the center today.
All classrooms were observed today.
Conversations were held with several staff and children.

The last emergency evacuation drill was made on 9/6/17. Shelter-in-place drill was made on 8/29/17.

Violations:
Standard #: 22VAC40-185-240-D-1-b
Description: Based on review of the staff's MAT (medication administration training) the class has not been updated within three years. Evidence: On 9/14/17, all staff that had MAT training had not updated the training within the required three year interval.

Plan of Correction: We will schedule the MAT training as soon as possible for staff.
