Comments:
An unannounced monitoring inspection was conducted beginning at 6:00 and ending at 7:40. There were 8 children present with 4 staff as of 7:15. The children were observed at arrival . Children and parents were greeted by staff. Children went to table activities at arrival.. Reviewed files for 5 staff - see violation notice Reviewed files for 7 children - complete Reviewed medication for 1 child - documentation complete Emergency escape drill and shelter in place drills are current as are Health and Fire Inspections. Standard #: 22VAC40-185-160-C
Description: Based on a review of 4 staff files, it was determined that 1 staff person did not have an up to date TB test. Evidence: Staff #2 had a TB test that expired on 8/11/17.

Plan of Correction: The Director will follow up with the staff person to assure that she has a current TB test/XRay within two weeks.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff files for 5 staff, it was determined that 1 staff person did not have a file on site. Evidence: Staff #5, who also works at a Middle School SACC does not have a file at this site.

Plan of Correction: The staff person will have a copy of her file on site at all times.
