Comments:
An unannounced monitoring inspection was conducted on September 13, 2017. There were three staff present with 31 school age children. Children were observed playing a variety of tabletop activities to include drawing and coloring, board games and card games, and talking freely amongst themselves and playing in the gymnasium. Five children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor areas, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation and procedures, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. One medication was reviewed in the center on this date. Exit interview conducted with the Recreation Supervisor (Staff #1).

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in three of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on September 13, 2017, the following documentation was missing from each child's record: 1. The record for Child #1 did not include documentation of complete addresses for two persons to contact in the event of an emergency where the parents cannot be reached. 2. The record for Child #3 did not include the complete address for one person to contact in the event of an emergency where the parent cannot be reached and the work phone numbers for both parents. 3. The record for Child #5 did not include the complete address for one person to contact in the event of an emergency where the parent cannot be reached.

Plan of Correction: Recreation Supervisor will contact the parents and have them complete the missing information.

Standard #: 22VAC40-185-540-E
Description: Based on observation and inspection of the facility, the licensee did not ensure that all required nonmedical emergency supplies were at the center. Evidence: During the inspection conducted on September 13, 2017, the battery operated radio was inoperable.

Plan of Correction: Recreation supervisor will purchase a new radio today.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on September 13, 2017, the findings of the most recent inspection conducted on June 27, 2017 were not posted. The facility had the findings of an inspection conducted on January 6, 2017 posted.

Plan of Correction: The most recent inspection will be posted today.
