A Subsidy Health and Safety inspection was conducted on September 13, 2017 to review supplemental health and safety requirements. Licensing Inspector arrived at 7:00am and departed at 9:00am to conduct a Subsidy Health and Safety Inspection. At the time of the tour, there were three staff present with 31 school age children. Children were observed playing a variety of tabletop activities to include drawing and coloring, board games and card games, and talking freely amongst themselves and playing in the gymnasium. Five children's records were reviewed on this date. Three staff records were reviewed on this date. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-004-F Description: Based on observation and inspection of the facility, the vendor did not ensure that a current list of all children's allergies, sensitivities, and dietary restrictions were posted with parental approval or kept in a way that is immediately accessible to staff. Evidence: The records for Child #1 and Child #3 were reviewed and there were documented allergies for each child that were not included on the center's allergy/inclusion binder. Plan of Correction: I will speak to the Inclusion Coordinator so that the grid can be updated based on the child's registration form.
