Thank you for your time and assistance with the unannounced monitoring inspection conducted September 7 and September 13, 2017. The on-site inspection was conducted on September 7, 2017 from 10:15 a.m. to 1:10 p.m. On this date, there were 33 children in the care of four (4) staff members in two (2) classrooms. There were a total of 37 children enrolled in the preschool program. Five (5) children's records were reviewed and four (4) staff members and five (5) children were interviewed. The medication and medication paperwork was reviewed for one (1) child. Four (4) staff members' records were reviewed on September 13, 2017 at the MACAA Head Start administration office. Lunch was served by the elementary school and was observed to meet USDA guidelines. Some of the items observed and reviewed included: current health and fire inspections, required posted material, documentation of emergency evacuation drills, updated emergency preparedness plan, centers, transitions, circle time, language art activities, story time, bathroom and hand washing procedures, lunch, and nap. Standard #: 22VAC40-185-450-A Description: Based on observation and verification from staff, the center failed to provide linens consisting of a top cover and a bottom cover on the rest mat for one child. Evidence: 1. In Classroom #2, one child was napping on a plastic mat that did not contain a bottom cover or a top cover. 2. Staff Member C reported that the child's parent did not send in linens for him to use during rest time, and she did not have extra linens in the classroom for children to use. Plan of Correction: The center obtained extra linens after school on 09/07/2017. They will be available for children to use if linens are not sent in one week.
