A Subsidy Health and Safety inspection was conducted on 09/18/2017 from 8:30am to 3:00pm to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on review, the center did not obtain a written allergy plan for each child with a diagnosed food allergy. Evidence: 1. Child A does not have a written allergy care plan and has documented allergies. Plan of Correction: I will ask parent to obtain an allergy care plan.
Standard #: VENDSUB-000-012-C Description: Based on review of staff records, one staff member did not complete the Virginia Preservice Training. Evidence: 1. Staff A has been employed with the center for more than 90 days and has not completed Virginia Preservice Training. Plan of Correction: I will ask the staff member complete it.
Standard #: VENDSUB-000-012-E-1 Description: Based on record review and interview, four staff members were not current in CPR Training. Evidence: 1. Staff A, Staff B, Staff C and Staff D are not current in CPR training and have been employed with this center for more than 30 days. Plan of Correction: I will set up a CPR class as soon as available and get them registered.
