Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:45am - 1:00pm. There were twelve preschool children in care with three staff. The children were observed playing on the playground, in group activities, during story time and preparing for lunch. Five children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-130-A
Description: Based on record reviews the center failed to obtain documentation of immunizations before children attended center. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (children's records #1 and #5) did not contain documentation of immunizations.

Plan of Correction: The teacher talked to the parent of child #5 during the inspection and requested a copy of the child's immunizations. The teacher will request a copy of child #1's immunizations from the child's parents.

Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. There were no addresses for the emergency contacts in all five records.

Plan of Correction: The teacher will contact the children's parents and request this information for the children's records.
