Technical Assistance:
An exit meeting was completed by the licensing inspector with the center's owner to review the findings of today's inspection.

The licensing inspector conducted an unannounced monitoring inspection on Tuesday, September 12, 2017 from 9:50 am to 2:00 pm to determine the center's compliance with licensing standards. During this inspection, the Director and the owner of the center were present and assisted the licensing inspector throughout this inspection. The census for today's inspection consisted of 25 children and six staff. The licensing inspector completed observations of the children on the first and second floor levels at the center. The infants were observed by the inspector in the designated area with staff playing with age appropriate toys on the floor. The Toddlers were observed by the inspector interacting with peers and staff, and playing with age appropriate toys. The children in the Twos group were observed by the licensing inspector participating in circle time with staff on the carpet, and lining up to transition outdoors to the nearby community park. The licensing inspector also completed observation of the staff completing head counts and providing instruction to the children. The Preschool children were observed by the inspector engaged in free play with various toys and equipment. During this inspection, the licensing inspector completed a formal observation of the Twos transitioning indoors, and preparing for lunch time in which the inspector observed the children completing bathroom breaks and handwashing. The inspector observed diapering by the staff of children in the Toddler and Twos groups. The diapering station was inspected for overall compliance with standards as well. The children's lunch consisted of turkey sandwich, salad, oranges, and milk.The inspector interacted with the staff and children throughout the inspection when appropriate. The children on the first and second floors were observed by the licensing inspector napping in the designated areas. The classrooms were equipped with age appropriate toys, and supplies. The staff were observed providing adequate supervision and addressing the children's overall needs. The center provides care to school aged children and they were not present during this inspection. During this inspection, the required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, lunch menu, license, injury reports, fire and shelter-in-place drills, liability insurance policy, and emergency preparedness plan. There were ten children?s records and seven employee records reviewed during this inspection. Medications is currently being administered at the center. The center provides transportation as well.
The center's first aid kit and emergency supplies were inspected.

Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records, the center did not ensure each staff member completed tuberculosis screenining within 21 days of employment.

Evidence: A review of staff#3 (date of hire: 6/19/17) was completed and there was no documentation included in the record for tuberculosis. There was an interview with staff and it was confirmed that no tuberculosis screening has been completed at this time.

Plan of Correction: A TB screening appointment will be scheduled for the staff member. The staff member will also be transported to the clinic by the Director to ensure completion.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's record, the center did not ensure a child's enrolled record contained all of the required information.

Evidence: A review of Child #1's record was completed and the emergency contact information listed was for the child's parent, therefore, there was only one complete emergency contact. The name, address, and contact information of two designated people to call in an emergency if parents cannot be reached is the standard.

Plan of Correction: Application will be reviewed and corrected by parent today.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the areas in the center, the center did not ensure all areas and equipment were maintained in a safe condition. Evidence: During this inspection, the inspector observed in the Twos/Threes area a hole in the wall and peeling paint areas accessible to the children in care.

Plan of Correction: The Lead Teacher will ensure the hole remains covered and inaccessible to children until building landlord/owner can fill hole and paint.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center did not ensure proper hand washing was completed during diapering.

Evidence: During this inspection, the licensing inspector observed two separate staff being noncompliant with proper hand washing procedures during diapering of children.
1. Staff was observed by inspector not washing their hands with soap and running water before and after helping a child during diaper change.

2. Two staff were observed by the inspector not completing children's hand washing with soap and running water after diapering.

Plan of Correction: Review of proper hand washing protocol will be done today with all assistant teachers. Observations will be done by Director and Lead Teacher.

Standard #: 22VAC40-185-500-B
Description: Based on observation at the center, the center did not ensure sight and sound supervision. Evidence: During this inspection, the licensing inspector observed a staff sweeping the floor with back turned while a child was already placed on the diaper table, then staff was observed resuming to diapering of the child. There shall be sight and sound supervision for all children when a child is being diapered.

Plan of Correction: The staff member has received additional hands on training regarding diapering requirements. The staff will be monitored and observed during spot checks by the Director and class Lead for a period of 14 days to ensure understanding and compliance.
