Comments:
An unannounced complaint inspection was conducted on September 6, 2017. The complainant reported concerns regarding staffing and supervision and special services. An inspection of the facility was conducted, documentation was reviewed and obtained and interviews were held with staff. Based on gathered information, there is not a preponderance of evidence to support the allegation, therefore this complaint has been determined to be NOT VALID. There were other violations found that were not reported. Areas of noncompliance are identified on the Violation Notice.

Description: Based on observation and inspection of the facility, the licensee did not ensure that the center shall prepare a document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center. Evidence: During the complaint inspection conducted on September 7, 2017, the center's vehicle used to transport children did not have a document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.)

Plan of Correction: Documents containing local emergency contacts, potential shelters, hospitals, etc. and routes will be printed and kept on van.

Description: Based on observation and inspection of the facility, the licensee did not ensure that the during the transportation of children the center shall have all required emergency numbers in the vehicle. Evidence: During the complaint inspection conducted on September 7, 2017, the center's vehicle used to transport children did not have a 911 or local dial number for police, fire and emergency medical services in the vehicle .

Plan of Correction: Police, fire and emergency number will be placed on the van.
