Comments:
An unannounced monitoring inspection was conducted on September 22, 2017 during the before care portion of the program. There were 25 children present at arrival to the facility with four staff members. The children were observed arriving to the program, playing with various tabletop activities, and participating in a share and show activity. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack consisted of yogurt and apples. There are no medications present in the center at this time. Five children's records were reviewed during the inspection. Four staff records were reviewed during the inspection. The exit interview was conducted with the Area Manager.

Violations:
Standard #: 22VAC40-185-150-B
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each medical report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. Evidence: During the inspection conducted on September 22, 2017, the immunization report in the record for Child #5 had not been signed by a physician, his designee, or an official of a local health department.

Plan of Correction: We are going to contact the parent today to obtain a copy.

Standard #: 22VAC40-185-550-B
Description: Based on review, the licensee did not ensure that the emergency preparedness plan shall contain all required procedural components. Evidence: During the inspection conducted on September 22, 2017, the emergency preparedness plan did not contain the following procedural components: 1. Emergency communication: Establishment of center emergency officer and backup officer to include 24-hour contact telephone number for each; notification of parents and local media. 2. Evacuation: secondary means of egress 3. Shelter-in-Place: inside assembly points; primary and secondary means of egress

Plan of Correction: Emergency and back up officers and phone numbers were corrected during the inspection. I will discuss with the staff regarding the above components and come up with a plan of how to implement those items.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection conducted on September 22, 2017, there were not shelter-in-place procedures posted in the center.

Plan of Correction: I will discuss with the site staff the procedures that need to be taken for the map and post them on the wall.
