Technical Assistance:
An exit meeting was completed by the licensing inspector with the director of the program to review today's inspection.

Comments:
The licensing inspector conducted an unannounced renewal inspection on Thursday, September 21, 2017 at approximately 12:50 pm until 3:06 pm to determine compliance with licensing standards. During this inspection, the director was present, assisted, and accompanied the licensing inspector throughout this inspection. Upon the inspector's arrival, the children in the early childhood program were transitioning from lunch. The licensing inspector completed a formal observation of the children sitting on the carpet watching a show, completing bathroom breaks, and preparing for nap time. The licensing inspector observed 10 children and 2 staff present in the designated classroom area. The inspector was informed that additional children participating in the program arrive at 3:10 pm, however, the maximum number of participants in the program is 20. The staff were observed by the inspector interacting with the children, providing age appropriate supervision, and assistance when needed. There were age appropriate toys and materials for the children observed throughout the classroom settings. The classroom, gymnasium, and designated areas for care were inspected by the licensing inspector. The required postings were reviewed and determined to be in compliance. The following information was reviewed: attendance log, emergency supplies/maps, fire and shelter-in-place drills, liability insurance policy, and emergency preparedness plan. There were five children?s records and five staff 's records reviewed during this inspection. Currently, there is no medication being administered. However, the center has multiple staff on site with MAT, First Aid/CPR, and daily health observation certifications. The center's first aid kit and emergency supplies were inspected.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the center did not ensure that staff resubmit TB test results every 2 years.

Evidence: In staff #4's record (date of hire 08/15), the last dated TB test was for 08/28/15. During an interview with staff, it was reported the staff completed the TB test, however, a copy of the screening was not retrieved for the staff record and could not be verified during this insepction. Staff resubmit TB test results every 2 years.

Plan of Correction: The center will ensure a copy of staff TB test is retrieved from the physician and included in the staff record.

Description: Based on observations at the center, the center did not ensure hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: During the inspection, the licensing inspector observed three containers of disinfectant wipes/cleaners placed on a storage locker in the designated area for the children in care.

Plan of Correction: Corrected on site.
