Comments:
An other inspection was conducted 7/10/2017 and 08/15/2017. The inspection was from a self reported incident that occurred in the 2 year old classroom. Interviews were conducted and one violation was cited from the incident.

Violations:
Standard #: 22VAC40-185-410-1
Description: Based on interviews, the center failed to ensure that forbidden action were not used according to the standards. Evidence: 1. Verified through interviews with staff that on June 1, 2017 staff # was help child # 1 to get ready for nap. Staff # 1 was patting the back of the child and the child did not want to sleep and stuck his tongue out at staff # 1 and tapped the child on the mouth. During nap, child # 1 did not want a top cover on him, staff # 1 went over to put the top cover back over child # 1 and he threw the blanket at the staff person. Staff # 1 tapped the child's hand and told him "no". No visible red marks for either taps.

Plan of Correction: Staff # 1 was released from the position at the center. All staff will be reminded of the forbidden actions.
