A Subsidy Health and Safety inspection was conducted on 09/27/17to review supplemental health and safety requirements. Licensing Inspector arrived at 9:45am and departed at 12:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 28 children in care with 10 staff present. Children were observed listening to music, during circle time, hand washing and during lunch. A sample of 2 staff and 4 children's records were reviewed. One medication and 1 over-the-counter skin product was also reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-B Description: Based on a record review and interview the vendor failed to ensure all staff had the required Virginia Preservice Training Child Care Staff. Evidence: Staff #1 (doe 10/01) did not have documentation of having received the Virginia Preservice Training Child Care Staff. Staff #1 stated she did not have the training. Plan of Correction: Staff #1 will have the preservice training complete by the end of the week.
Standard #: VENDSUB-000-012-D Description: Based on a record review the vendor failed to ensure all required components were included in the orientation of staff. Evidence: The center had 3 infant staff. Neither staff had documentation of having received orientation in shaken baby syndrome or safe sleep. Plan of Correction: The owner will ensure all staff receive an orientation on shaken baby syndrome and safe sleep. It will be documented in each staff's file.
Standard #: VENDSUB-000-031-A Description: Based on a record review the vendor failed to ensure written emergency plans included all required components. Evidence: The center's emergency plan did not include procedures for an intruder or reunification plans for an evacuation, shelter-in-place or intruder procedures. Plan of Correction: The center does have procedures for an intruder and reunifications plans, however it is not in writing. The owner will add these components to the plan and review will all staff.
