22VAC40-191 Background Checks (22VAC40-191)
- Technical Assistance:
The following was discussed with the owner: A one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center. It was recommended that additional staff are trained in MAT. While there is a staff present currently scheduled at all times there is a child in care with medication, however, if that status changes it would be a good idea to have a back-up.
Licensing Inspector arrived at 9:45am and departed at 12:00pm to conduct a monitoring Inspection. At the time of the tour there were 28 children in care with 10 staff present. Children were observed listening to music, during circle time, hand washing and during lunch. A sample of 2 staff and 4 children's records were reviewed. One medication and 1 over-the-counter skin product was also reviewed.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: In the boys bathroom the flooring around the toilet had stains and there was rust on the side of the toilet base and on the floor. Plan of Correction: The owner will replace the tiles and remove the rust.
Standard #: 22VAC40-185-430-L Description: Based on observation the center failed to ensure infant walkers were not used. Evidence: In the infant room there was a metal infant walker. The staff stated one of the infants uses the walker. Plan of Correction: The infant was diagnosed with delays and the parent brought the walker in for therapy. The owner will ask the parent for a doctor's note and instructions for the piece of equipment.
Standard #: 22VAC40-185-440-H Description: Based on observation the center failed to ensure all requirements were met for cribs. Evidence: An infant was observed sitting in her crib with the mattress curled up on the sides, creating more than an inch of space between the mattress and the crib and exposing the bottom. Plan of Correction: The sheet was placed on the mattress incorrectly. The owner will talk with all infant staff about ensuring sheets are on correctly before placing children in the crib.
Standard #: 22VAC40-185-550-D Description: Based on a record review the center failed to implement a monthly practice evacuation drill. Evidence: The log the center uses to document fire drills did not have a drill listed as being done in August 2017. Plan of Correction: The owner stated the drill was done, however, she failed to document it. The owner will document the date in the log and will conduct a drill before the end of September.
Standard #: 22VAC40-191-40-D-1-C Description: Based on a record review the center failed to ensure all required repeat background checks were complete before three years since the last one. Evidence: Staff #1's last criminal record check was dated 12/01/11. Plan of Correction: The criminal record check was sent out in July, however, it was sent back. It was resent in August and the provider is waiting for the results to be returned.
