Thank you for your assistance during this unannounced inspection. This on site visit was conducted on September 6, 2017 from 11:30 a.m. through 1:30 p.m. as a result of a self-reported incident by Bright Beginnings Administration on August 24, 2017. On August 24, 2017 two children, two years of age, were out of sound supervision for approximately thirty seconds. Areas of non-compliance may be found on the violation notice. Description: Based on the interview with four staff members, two children, age two, were out if sight supervision for approximately 30 seconds. Evidence: 1. On 08/24/2017, Staff A had five children outside on the toddler fenced in playground. Staff B had four children inside the classroom. 2. Two children, age two, "squeezed" out the gate and turned left to continue on the sidewalk. 3. Staff A opened the classroom door to tell Staff B two children were outside the gate of the playground and walking down the sidewalk. 4. Staff B told Staff A to stand between the classroom door and the playground to watch the other children while she ran out the gate to retrieve the two children. 5. Staff C, D, and E were in the office viewing the outside cameras and saw the two children on the sidewalk. 6. Staff C, D and E ran out of the entrance doors to the sidewalk to retrieve the two children. 7. The two children were out of sound supervision for approximately 30 seconds.

Plan of Correction: Staff member supervising children was 9 months pregnant and could not run so immediately alerted co-worker to secure children which was done in under 30 seconds. Staff member was immediately terminated. All staff were retrained on playground safety including site and sound supervision and continually scanning playgrounds. Gate was immediately double secured using zip ties.
