Comments:
An unannounced other inspection was conducted on 09/15/2017 in response to a self-report incident regarding an allegation of inadequate supervision. Based on the information gathered the allegation is found to be not valid. One violation was observed in the area of injury documentation. The violation was reviewed with the program director at the exit interview and is listed on the violation notice.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center failed to ensure that the written record of children's injuries shall include staff and parent signatures or two staff signatures. Evidence: The written incident report for an injury received by a child on 09/07/2017 lacked a parent signature or two staff signatures.

Plan of Correction: The Director will review the policy with all staff at our monthly staff meeting and reminders in our daily announcements. The entire team will check for the required amount of signatures.
