Comments:
An unannounced renewal inspection was conducted today from 8:35am-11:30pm. There were 30 (infants-5yrs.) directly supervised by 10 staff. The physical plant, 7 staff records and training records, 5 children?s records, no children's medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, participating in group play, individual activities, circle time and eating breakfast. Diapering and hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-40-J
Description: Based on review, the facility failed to ensure that Injury Prevention Procedures were updated at least annually, based on documentation of injuries and a review of activities and services. Evidence: Reviewed the injury reports and found that there was no documentation of an annual review of injuries and accidents.

Plan of Correction: We're going to do them monthly so that we are in compliance with accreditation and licensing.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that they were obtaining annually updated/reviewed emergency contact information. Evidence: Reviewed 5 children's records and found that CH3 and CH5 did not have annually reviewed/updated emergency contact information.

Plan of Correction: Will have parents fill that out by Friday.

Standard #: 22VAC40-185-190-A
Description: Based on review, the facility failed to ensure that they obtained documentation of Program Director qualifications. Evidence: Reviewed the director's records and found that there was no documentation of Program Director qualifications.

Plan of Correction: Will be able to have that in the file by Friday.

Standard #: 22VAC40-185-240-D-5
Description: Based on review, the facility failed to ensure that there was one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: Reviewed 7 staff training records and found that there was no documentation of Daily Health Observation.

Plan of Correction: Those will be done by the end of the month.

Standard #: 22VAC40-185-270-A
Description: Based on review, the facility failed to ensure that the areas and equipment of the center, inside and outside are maintained in a clean, safe and operable condition. Evidence: 1) Observed in the Infant 1 room, near the far corner of the play area, peeling paint, accessible to children. 2) Observed in the Toddler Room, near the hanging mirrors, peeling paint accessible to children. 3) Observed a stuffed toy in the Infant 2 room that had a hole in the "hand" that was exposing the inside stuffing. 4) Observed in the Twos Room, a stuffed round toy where the "plastic" material had ripped off and exposed the stuffing material.

Plan of Correction: Toys were removed during the inspection. Will have maintenance come out and deal with the paint.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the facility failed to ensure that unused electrical outlets were covered with protective covers. Evidence: Observed in the Twos bathroom, an uncovered electrical outlet.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the facility failed to ensure that there was resilient surfacing under climbing equipment and around all fall zones surrounding the equipment. Evidence: Observed that underneath a climbing structure that was over 6 feet in height, that there was no resilient surface underneath, only astro-turf, which did not have any resiliency of any kind.

Plan of Correction: Will submit a request to maintenance today and will keep the equipment off limits until it is corrected.

Standard #: 22VAC40-185-550-M
Description: Based on review, the facility failed to ensure that injury/accident reports contained documentation of "any future action to prevent recurrence". Evidence: Reviewed 5 injury accident reports and found that there was no documentation of "any future action to prevent recurrence".

Plan of Correction: Will add to reports going forward from today.
