1. A Subsidy Health and Safety inspection was conducted on October 4, 2017 to review supplemental health and safety requirements. Twenty five children were present in four classrooms with direct care staff in ratios for supervision. Rooms are well maintained and equipped. Observed infant care, transition from lunch, free play and staff preparing babies for a walk around the neighborhood. All staff records were reviewed to verify VA Pre-Service Training as well as first aid and CPR certification has been completed. No medication was reported to be on-site for children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-031-B-3 Description: Based on staff interview and review of the written emergency plan, a component to reunify parent with their child is not included for relocation procedures. Evidence: A component of the written emergency plan does not include procedures to reunite the child with the parent or designated guardian following a relocation emergency. Plan of Correction: The Center Director will develop a written plan to reunite the family at the local grocery store which is within walking distance to the center. This will be added to the parent handbook.
