Technical Assistance:
A Subsidy Health and Safety inspection was conducted on 10/12/17 from 10:00am until 2:00 pm to review supplemental health and safety requirements. At the time of the inspection there were 29 children in care, ages 3 months to preschool, with 6 staff. 11 school age children arrived with 1 staff during the course of the inspection. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Violations were discussed with the vendor during the exit interview.

Description: Based on observation and interview, the vendor did not ensure that the emergency preparedness plan's lockdown procedures shall include procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. Evidence: 1. The emergency preparedness plan's lockdown procedure did not contain a plan for reunification of children with with their parents. 2. The center director confirmed that the lockdown procedure did not contain a reunification plan.

Plan of Correction: The vendor will ensure that a reunification plan is added to the lockdown procedures.
