An unused toddler slide was observed over a non resilient surface. The center has stated that this equipment is not used and it's removal was recommended by the inspector. Topical Ointment forms should clearly specify the specific name of the product to be applied and not very generic descriptors such as "Sunscreen".
An unannounced monitoring inspection was conducted today from 12:15pm-1:40pm. There were 91 children directly supervised by 15 staff. The physical plant, staff records, children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications present during the inspection. No new staff have been hired since the last inspection. Infants were observed being fed and other children in the center were observed to be napping. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A Description: Based on observation and staff statements, not all areas of the center were maintained in a clean safe and operable condition. Evidence: 1. In the infant room a plastic bag used for storage of flat paper items was observed between the mattress and base of a crib assigned to a child in care. The bag riding up the side of the crib mattress in such a way that a child placed in the crib would be able to tug on the edge of the bag posing a choking or suffocation hazard. 2. One mat in the toddler room was observed with two large holes which exposed the interior foam padding. 3. The toilet adjacent to classroom D was observed to have the base of the stop valve covered in rust, and the supply tube thickly covered with dust and grime. The base of the toilet was observed to have significant rust and grime stains. Plan of Correction: Not available online. Contact Inspector for more information.
