Comments:
An unannounced monitoring inspection was conducted from 7:15 am - 9:00 am. There were 45 children in care being supervised by 4 staff members. The children were observed participating in crafts, structured center play, cooperative games, and eating breakfast. Breakfast served today is Pancakes, cereal, egg pop, oranges, milk, fruit juice. Ten children's records were reviewed and in compliance. Four staff records were reviewed and in compliance. Five medication's were reviewed. There are currently two staff members on site who are MAT (Medication Administration Training) certified. There are four staff members on site who are CPR and First Aid certified. The most recent emergency escape drill was conducted 09/28/2017 and the most recent shelter in place drill was conducted 09/26/2017. Standard #: 22VAC40-185-510-A
Description: Based on review of five medications it was determined the center failed to follow all procedures for administering medication. Evidence: The medication authorization form for Child A dated 08/22/2017 was not signed by the parent.

Plan of Correction: Administration will request the parent signature of child A for the authorization form.
