Comments:
The inspector conducted an unannounced monitoring inspection from 11:45 until 1:00 a.m. There were 15 children in care upon arrival with three staff members providing supervision at all times. Children were observed playing outside, having a bathroom break, and getting ready for a nap. Five children's records were reviewed. Medications were inspected. The violation is identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-270-A
Description: Based on observation and measurement, the center did not maintain an outside area in a safe condition. Evidence: 1. The inspector walked up the steps to the upper ball field where the children were playing and there were two steps that were not level. The 13th step going up had a one inch gap between the two boards creating a tripping hazard and the 15th step from the top had a one inch gap between the two boards. The 12th step was loose.

Plan of Correction: The director will have the steps fixed. The center is going to reevaluate using the steps leading up to the ball fields. They are only used on occasion.
