63.2 Child Abuse and Neglect

Comments:
An unannounced monitoring inspection was conducted on Thursday, October 19/2017, from approximately 10:30am until 4:10pm. The following classroom child to staff ratios were observed: Infant A - three children with one staff Infant B- ten children with three children Toddlers- twelve children with three staff Twaddler- eight children with one staff Prepper- twenty-one children with three staff Preschool 1 - ten children with one staff Preschool 2 - ten children with one staff Pre-K 1 - nineteen children with two staff School age - eleven children with one staff Children were involved in a variety of activities. Staff were observed interacting frequently with children. Activities such as singing songs, discussing the weather and days of the week, coloring and discussing healthy foods were observed. Lunch: Ham and cheese with wheat bread, corn, fruit cocktail and milk PM Snack: Applesauce and milk Fifteen of twenty-two staff are currently certified in first aid and CPR. Five staff are currently certified in Daily Health Observation. Four staff are currently certified in Medication Administration. The last practice evacuation drill was conducted 9/29/17. All required emergency supplies were present. Ten children's records and ten staff records were inspected. Medications for children were also inspected.

Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the center did not ensure each staff person submitted a follow-up tuberculous screening at least every two years. Evidence: Staff # 6 tuberculous screening expired 8/25/17.

Plan of Correction: Per Center: Staff member will be sent tomorrow to have TB test completed.

Standard #: 22VAC40-185-70-A
Description: Based on inspection of staff records, the center did not ensure each record contained documentation of two references prior to employment. Evidence: Out of ten staff records inspected, Staff #1 began working 9/1/17 and does not have two references. Staff #9 began working 8/11/17 and does not have two references. Staff # 8 began working 8/25/17 and has one of the two required references.

Plan of Correction: Per Center: During inspection, references were called, but was unable to speak with anyone live. Center Director will follow up tomorrow to ensure proper references are included in file.

Standard #: 22VAC40-185-500-A
Description: Based on observations, the center did not ensure staff washed their hands after changing each child's diaper. Evidence: In the Infant B classroom, a staff person was observed changing diapers. The staff person did not wash their hands between diapering the first, second or third child.

Plan of Correction: Per Center: Center Director went into the Infant B classroom during inspection to give coaching session to the staff member. Center Director will hold an Infant team meeting to ensure all staff are properly trained on diaper changing procedures.
