Comments:
Today an unannounced Monitoring Inspection took place between the hours of 9:55am and 11:45am. At the start of the inspection there were a total of 13 children with 2 staff in the preschool classrooms, and 19 children with 2 staff in the Kindergarten and 1st and 2nd grade classrooms. A total of 5 children?s files and 2 staff files were reviewed. The children were observed eating their morning snacks, practicing writing the letter 'M' and going outside to play. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review and discussion, each child did not have a physical exam on file within the first 30 days of attendance. Evidence: Child #1 and Child #2 did not have physical exams in their file. Manager confirmed that both children have been enrolled for more than 30 days.

Plan of Correction: We have emailed the parents asking for these forms. We will email them requesting it.
