22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 11:30am - 1:20pm. Upon arrival there were 82 children in care with 22 staff present. A sample of 8 children and 3 staff records were reviewed in addition to 4 medications. Children were observed on the playground and during nap. Infants were observed playing on the floor and being fed by staff.
Standard #: 22VAC40-185-280-B Description: Based on observation the center failed to ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: The chemical cabinet was unlocked and open. There was a bottle of disinfectant sitting on the counter top. All three staff were engaged with the children and not using the cleaning products. Plan of Correction: The director will talk to the infant staff about keeping chemicals locked at all times.
Standard #: 22VAC40-185-510-G Description: Based on a record review the center failed to ensure all requirements were met when agreeing to administer prescription medication. Evidence: There was an emergency medication at the center that did not match the dosage prescribed by the doctor. The dosage on the medication was .3mg, however the authorization by the parent and doctor stated .15mg. Plan of Correction: The staff member in charge of medication will send the authorization home today with the child's parent to obtain the correct authorization form.
Standard #: 22VAC40-185-570-I Description: Based on observation and interview the center failed to ensure there was a one-day's emergency supply of all required items. Evidence: The infant room did not have water to activate the powdered formula to be used in an evacuation or emergency. Plan of Correction: The director will ensure the infant room has enough water for a one day's supply of powdered formula.
