Comments:
An unannounced renewal inspection was conducted today. The inspector arrived at 12:00 PM and departed at 3:30 PM. Twenty-one children were in care upon arrival. Nap time, afternoon snack, activities and arrival of the school aged children were observed. A sample size of six staff records and five children's records were reviewed. Medication administration and injury records were reviewed. AN exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes the names, addresses and telephone numbers for two designated persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The record provided for child 1 includes the cities only (no street addresses) for the two designated emergency contact persons. 2. The records provided for child 2 and child 3 do not include addresses for one of the two designated emergency contact persons. 3. The record provided for child 4 does not have the city/state or zip code in the addresses for the two designated emergency contact persons. 4. The record provided for child 5 does not include the street address for one of the two designated persons to be contacted in an emergency. 5. Staff 1 was unable to provide the required information in the children's records listed above.

Plan of Correction: The provider responded with the following: All children's records will be checked to ensure that complete addresses are included for designated emergency contact persons.

Standard #: 22VAC40-185-510-A
Description: Based upon review of the medication stored at the facility for administration to children, the written parental authorizations and staff interview, the facility has not ensured that they have followed their own policy regarding parental authorizations. Evidence: 1. The written parental authorization for the administration of a ventolin inhaler to child 4 expired 10/25/2017. 2. The written parental authorization for the administration of diphenhydramine to child 4 expired 10/2/2017. 3. The written parental authorization for the administration of an epi-pen jr. to child 4 expired 10/2/2017. 4. Staff 1 was unable to provide updated parental authorizations for the above medications.

Plan of Correction: The facility responded with the following: New parental authorizations will be obtained. A tracking sheet will be created to ensure that authorization expiration dates are tracked.

Standard #: 22VAC40-185-550-D
Description: Based upon review of evacuation drill records, the facility has not ensured that they have implemented monthly evacuation drills. Evidence: The practice evacuation drill log provided does not have documented an evacuation drill in July 2017.

Plan of Correction: The facility responded with the following: Evacuation drills will be conducted and documented monthly.

Standard #: 22VAC40-185-550-M
Description: Based upon review of written injury records, the facility has not ensured that written injury records include all required documentation. Evidence: 1. An injury record dated 9/7/2017 for child 6 does not have documented the time or how the parent was notified of the child's injury. 2. An injury record dated 9/21/2017 for child 6 does not have documented the time or how the parent was notified of the child's injury.

Plan of Correction: The facility responded with the following: All written injury records will be carefully checked to ensure that all required information is documented. Staff will be reminded of required documentation.

Standard #: 22VAC40-191-60-C-1
Description: Based upon review of six staff records and staff interview, the facility has not ensured that they have obtained an original criminal history record report within 30 days of employment or continued employ is denied. Evidence: 1. The record provided for staff 5 indicates a hire date of 9/20/2017. The record does not include a criminal history record report. There was no documentation of follow-up with the VA state police regarding the requested criminal history record. 2. Staff 5 was on duty with the children during today's inspection. 3. Staff 1 was unable to provide documentation of follow-up with the VA state police for staff 5.

Plan of Correction: The facility responded with the following: The check for the criminal history record request has been cashed. The program director will follow up with the state police to determine if the request has been processed. All follow up will be documented.

Standard #: 63.2-1720.1-A
Description: Based upon review of six staff records, the facility has not ensured that they have not hired for compensated employment, continue to employ, or permit to serve as a volunteer who will be alone with, in control of, or supervising children any person who (i) has been convicted of any barrier crime as defined in ? 19.2-392.02. Evidence: The Virginia state police criminal history record provided for staff 7 indicates a felony conviction in 2014. Staff 7 has been working with the children since the facility was licensed in May 2017.

Plan of Correction: The facility responded with the following: Staff 7 will be terminated from employ immediately. In the future, criminal history records will be carefully examined to ensure that any staff with convictions that are barriers to working with children are not employed.
