The licensing inspector conducted an unannounced monitoring inspection on 10/31/17 in response to a self report that was received in the licensing office on 10/25/17 regarding allegations in the area of programs. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegation, so the complaint is determined to be VALID.
Standard #: 22VAC40-185-410-1 Description: Based on a self report, a record review and interview with staff the center failed to ensure staff did not use forbidden actions with children in care.
On 10/23/17 Child #1 took blocks from another child. In response Staff #1 took child #1's hands and placed them in an X shape and hit the child's top hand.
a. That afternoon child #1 told staff #2 , "Nan," Staff #1 "hit." Staff #2 stated when asked, Staff #1 said, "I pop him like this" Staff #1 took staff #2's arms and crossed them and hit the top of the hand.
b. In an interview conducted by staff #2 #3, #4 and #5 staff #1 admitted to hitting child #1 on the hand.
c. Staff #3 reported Staff #1 stated "I took his hand and tapped him."
d. In an interview with child #1's mother, it was stated child #1 came home a few times and said he was hit by Staff #1. The mother confirmed staff #1 admitted hitting the child #1 on the hand.
Plan of Correction: Staff #1 was placed on administrative leave and then terminated on 10/25/17. The incident was reported to licensing on 10/25/17.
