Comments:
An unannounced monitoring visit and facility tour was conducted from 2:15 pm-4:05 pm. At the time of inspectors' arrival there were 78 toddler to preschool aged children in care with 11 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, transitioning between activities and toileting and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance were discussed with the center administration at the conclusion of this inspection and are detailed on the violation notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was peeling drywall in staff #6's bathroom in the class and in the gym. 2. There was exposed metal on the corner of the bathroom wall in staff #6's bathroom. 3. There was peeling paint on the window sill by the book area and gray/green debris in the window sill by the back door in staff #1's room. 4. There was peeling paint and exposed rust on the ladder rungs and hand rails on the outdoor climbing equipment. The above items were accessible to children in care and could be potential injury hazards.

Plan of Correction: The areas noted during the inspection will be repaired or cleaned.

Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined that the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: The staff closet in the hallway was unlocked and contained multiple cleaning products that carry the label "keep out of the reach of children".

Plan of Correction: The staff closet was locked during the inspection. Staff will be reminded to keep this locked at all times.

Standard #: 22VAC40-185-290-3
Description: Based on observation it was determined that the center did not ensure that all areas utilized by preschool aged children had protective covers on all unused electrical outlets. Evidence: There were two uncovered outlets on a surge protector in staff #7's room. There were preschool aged children in this room during the inspection.

Plan of Correction: Outlets were covered during the inspection.
