Technical Assistance:
Discussed sanitizing and hand washing procedures. Provider is required to send to Licensing Inspector within 10 business day documentation of submitting new backgrounds checks for those staff with expired checks.

Comments:
An unannounced Monitoring Inspection was conducted on October 25, 2017 from approximately 10:40 a.m. until 3:00 p.m. Upon entrance there were 22 children in care and 5 staff present with children. The record review sample consisted of 5 children's records and 5 staff records. Children were observed eating lunch, preparing for and during rest period and playing in centers to include blocks and imagination. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Standard #: 22VAC40-185-260-A
Description: Based on documentation review and staff interviews, the licensee did not ensure an annual fire inspection report from the appropriate fire official having jurisdiction was obtained. Evidence: During the inspection on October 25, 2017, the most recent fire inspection was dated 8-29-2016 .

Plan of Correction: The center director will call and get inspection.

Standard #: 22VAC40-185-440-H
Description: Based on observation and staff interviews, the licensee did not ensure there was no more than one inch between the mattress and the crib. Evidence: During the inspection on October 25, 2017, child #3, a infant was sleeping in a crib and the front right corner of the mattress was folded under the sleeping infant and there was 2 inches between the mattress and the crib.

Plan of Correction: The center director will contact the owner and purchase better mattress.

Standard #: 22VAC40-185-440-L
Description: Based on observation and staff interviews, the licensee did not ensure pillows and filled comforters were not be used by children under two years of age. Evidence: During the inspection on October 25, 2017, child #4, an infant was observed sleeping with a boppy pillow under their neck and shoulders in the crib.

Plan of Correction: The center director removed the boppy during inspection.

Standard #: 22VAC40-185-560-F-4
Description: Based on measurements and observation, the licensee did not ensure children three years of age or younger were not offered foods that are considered to be potential choking hazards. Evidence: During the inspection on October 25, 2017, child #1 and child #2, both age 2 were observed having a banana that measured 3 inches long and was not cut in a manner to avoid potential choking hazard. Foods that are round, hard, small, slippery, or thick and slippery are considered high-risk foods for young children because they can lodge in small airways causing a child to choke.

Plan of Correction: The center director will ensure the teachers know they need to cut the food up into smaller pieces and the kitchen will cut it up as needed.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on documentation review and staff interviews, licensee did not ensure 2 of 5 staff had obtained an updated sworn statement or affirmation, central registry finding (CPS) and criminal history record check (CRC) before three years since the dates of these checks. Evidence: During the inspection on October 24, 2017, the record for staff #1 did not include documentation of updated sworn statement, the one available for review was dated 12-12-13. The record for staff #2 did not include updated sworn statement, CRC and CPS. The current ones available for review are dated as follows; CRC dated 5-27-14, sworn statement dated 6-18-13 and CPS dated 5-20-14.

Plan of Correction: The center director will continue to a check all three dates of the backgrounds check and will send them off. The checks are sent off annually in October and November.
