Technical Assistance:
Discussion of an alleged incident that occurred on Friday, October 27, 2017, between the times of 4:15 pm through 4:30 pm. This incindent was then self-reported to the Division of Licensing.

Comments:
The following inspection took place between the hours of 10:30 am through 11:30am. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interview, the operational responsibilities of the licensee did not ensure that the center's activities were maintained in compliance with these standards, the center's own policies and procedures that were required by these standards, and the terms of the current license issued by the department. Evidence - 1. The Assistant Director stated (during an interview conducted on the date of inspection - 11/06/2017) that Staff #1 failed to follow center policy by not conducting a name-to-face roll call once her class exited their classroom and arrived on the center playground. 2. Child #1 was left alone in her classroom for a total of 12 minutes without direct sight and sound supervision.

Plan of Correction: A professional training day will occur this Friday. This training will cover face-to-name procedure. Administrative staff will monitor several times a day to ensure compliance. Staff #1 was terminated from employment.

Standard #: 22VAC40-185-340-F
Description: Based on interview and written statements, a child under 10 years of age was not always within actual sight and sound supervision of staff. Evidence - 1. On the date of inspection (11/06/2017), it was determined that on Friday, October 27, 2017, Child #1 was left alone and unsupervised within the Pre-K 1 classroom for a total of 12 minutes. 2. Staff #1 took a pre-school class from their classroom out to the center playground. According to Center Administration, based on their observation of the video tape recording of this classroom and talking to staff, Staff #1 failed to conduct a name to face attendance check once outside with the children. 3. According to the video, Child #1 stepped back into the classroom alone, and was there for a duration of 12 minutes (4:15 pm to 4:27 pm). 4. Child #1 was found by Parent #1 (a parent of another child who later came into the room) and alerted staff. 5. Staff #2 escorted Child #1 to the playground; returning her to Staff #1 at 4:30 pm.
