An unannounced monitoring inspection was completed on 11/13/2017. There were 31 children, ages 4 years-11 years and 3 staff providing direct supervision present during today's inspection. There were 2 additional bus drivers present to transport to children from school to the Y. The inspector reviewed 5 children?s records, 5 staff records and 3 medications today. The children were observed in the following activities: homework time, arrival (the children arrive at various times via YMCA buses), afternoon snack and participating in Kung-Fu. Proper hand washing and table sanitization were also observed. The inspector discussed the following with the provider: adding a before school program, adding procedures for a lockdown, daily outdoor play, and annual training hours. An exit conference took place at the conclusion of today's inspection. The inspector arrived at 2:35 p.m. and departed at 5:10 p.m. Thank you for your time today. Standard #: 22VAC40-185-60-A
Description: Based on record review, the facility failed to ensure that each child's record contained all of the components required by the standards. Evidence: 1. The record for Child #1 did not contain the names of persons authorized to pick up the child. 2. The record for Child #2 did not contain the following information: allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation and the first date of attendance.

Plan of Correction: The Child Care Director will get the information from parents.

Standard #: 22VAC40-185-380-A
Description: Based on observations and discussion with staff, the facility failed to ensure that there was a posted daily schedule to include opportunities for outdoor activities, weather and air-quality permitting. Evidence: 1. There was no posted daily schedule on-site; the Child Care Director was not aware that a posted schedule was needed. 2. The program provides after school care to preschool children.

Plan of Correction: The Child Care Director will develop and post a daily schedule.
