An unannounced renewal study was conducted today. 6 children's records and 5 staff records were reviewed. See violation notice form missing information. Children were observed in a variety of activities today. The play ground was not inspected due to inclement conditions. A Subsidy Health and Safety Inspection (SHSI) was conducted today as well. One additional violation was added regarding background searches for the Board officers.

Description: Based upon a review of records, children's records are incomplete. Evidence: 3 of 5 records reviewed was missing information. Missing information includes, but may not be limited to: *Parent work information (Children # 1, 4) *Complete Emergency contact information (Child #3)

Plan of Correction: Parents will be asked to complete the missing information.

Description: Based upon a review of documentation, there is no staff person current in training for Daily Health Observation.

Plan of Correction: I will have multiple staff sign up for the online training.

Description: Based upon observation, hazardous substances were not kept locked while not in use. Evidence: Cleaning products were stored on top of the refrigerator in the "CareBears" room.

Plan of Correction: Staff will be reminded to lock up the hazardous materials.

Description: Based upon direct observation, one classroom as out of the ratio of 1staff to 10 children. Evidence: The Zebra's classroom (Ages 4/5 years) was let out of ration when a staff member left the room to take a medication to the front office. There were 14 children and 1 staff member present.

Plan of Correction: Staff will be reminded to never step out of the room, leaving it out of ratio.

Description: Based upon are view of documentation, parents have not been provided the amount of time infants have spent on their stomachs, on the daily record. Evidence: 3 of 3 children required to spend time on their stomachs, have not had 30 minutes of tummy time documented on the child's daily sheet.

Plan of Correction: Staff will be reminded to document 30 minutes of tummy time daily.

Description: Based upon a review of medications, one medication authorization is incomplete. Evidence: Child #5' medication authorization for Albuterol did not contain the parent's signature.

Plan of Correction: I will ask the parent to complete the form

Description: Based upon a review of medications, one medication authorization has expired. Child #6's authorization for medication expired 10/22/2017.

Plan of Correction: I will return the medication to the parent.

Description: Based upon observation, medication was not stored locked. Evidence: The medication box in the Zebra's classroom was not locked, nor stored in a locking cabinet.

Plan of Correction: I will remind the staff to lock medications.

Description: Based upon a review of documentation, the emergency preparedness plan does not include all required components. Evidence: The plan available for review does not contain the components of relocation information.

Plan of Correction: We have the components, they are just not written down. They will be written down and given to the fire marshal for review.

Description: Based upon direct observation, bottles are not dated. Evidence: 3 of 4 bottles reviewed did not contain the and date for the bottle. 1 of 4 did not contain the child's name or date.

Plan of Correction: Bottles will be labeled and dated.

Description: Based upon a review of documentation, background searches are not available for the Board officers. Evidence: Current Sworn Disclosure statements, Criminal Records Checks and Central Registry searches are not available for the listed officers.
