An unannounced monitoring inspection was conducted on 11/13/17 from 9:30am - 11:30am. During the inspection there were 56 children ages two years old to five years old in care with 17 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms and eating morning snack. Records were reviewed for six children and six staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.
Standard #: 22VAC40-185-160-A Description: Based on a review of six staff records, it was determined that the facility did not ensure that each staff member submits documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employment begins. Evidence: 1. The record for staff #6 (date of hire 9/6/17) contained documentation of a negative TB screening that was dated 11/9/17. 2. Administrative staff reviewed the record for staff #6, and confirmed that the TB screening was completed more than 21 day after employment. Plan of Correction: The facility responded: Center management will ensure that each staff member gets their TB test completed within the first 21 days of employment.
Standard #: 22VAC40-185-70-A Description: Based a review of six staff records, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each staff that contains all required information. Evidence: 1. The record for staff #5 did not include documentation for the name, address, and telephone number of a person to be contacted in an emergency. 2. The record for staff #6 did not include documentation for the name, address, and telephone number of a person to be contacted in an emergency. 3. Staff #1 (Program Director) confirmed that the records for staff #5 and staff #6 were not complete. Plan of Correction: The facility responded: Both staff will update their records with the information for an emergency contact. All new staff will have a documented emergency contact prior to starting employment.
Standard #: 22VAC40-185-550-M Description: Based on a review of 35 injury/accident reports, it was determined that the facility did not ensure that the written record of children's serious and minor injuries included all required items. Evidence: 1. Four of the injury reports reviewed did not include any future action to prevent recurrence of the injury. 2. Four of the injury reports reviewed did not include the method of how the parents were notified of the injury. Plan of Correction: The facility responded: Teachers will be re-trained on how to complete an injury report to ensure all required items are completed. Center Management will review all injury reports to ensure they are completed correctly.
