Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 15, 2017. The on-site inspection was from 8:35a.m. to 12:10p.m. On this date, there were nine (9) children present and in the care of two (2) staff members. There were a total of 13 children enrolled in the morning preschool program. The violation from the previous inspection was reviewed for compliance. The exceptions to compliance were noted on the violation notice and the risk ratings were documented on the supplemental page. There were five (5) children's records reviewed and two (2) staff records reviewed. There were two staff members interviewed, as well as two children. There were no medications being administered at the time of the visit which is the policy of the center. Snack was provided by the center and consisted of pretzels, graham crackers, and pineapple. Theme for the week was Turkeys and the story of the week was The Little Red Hen. Some items reviewed and observed include: required posted information, injury report, first aid kit and emergency supplies, documentation of evacuation and shelter-in-place drills, arrival procedures, free choice time, hand washing and bathroom procedures, snack, diapering procedures, a puppet show to learn about pilgrims and the first Thanksgiving, story time, outdoor play, musical activities, and painting. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on the review of five children's records on 11/15/2017, the center failed to obtain documentation that two children had been adequately immunized before the children's first day of attendance. Evidence: 1. Documentation of Child C's religious exemption for immunizations was received by the center on 09/08/2017. First date of attendance for Child C was 08/23/2017. 2. Documentation of Child E's immunizations was received by the center on 08/25/2017. First date of attendance for Child D was 08/22/2017.

Plan of Correction: In the future, the director will ensure that documentation of immunizations has been obtained before a child can attend the center.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of two staff records on 11/15/2017 and verification from the director, the center failed to obtain a central registry finding for one new employee. Evidence: 1. The record for Staff #2 did not contain a completed search of the central registry. Date of hire for Staff #2 was 08/21/2017. 2. The director verified that a search of the central registry request was sent but it did not contain a finding upon return.

Plan of Correction: The director will resend the request for a search of the central registry as soon as possible and inform the licensing inspector as soon as it has been mailed.
