A Subsidy Health and Safety inspection was conducted on 11/15/2017 to review supplemental health and safety requirements. Today an unannounced monitoring inspection was conducted between the hours of 2:20pm and 4:30pm. I observed 54 children with 3 staff members. Ratios and supervision were in compliance. Children were observed arriving to the program, eating snack, doing homework, art projects, Origami, reading books and playing table games. Staff were actively assisting them with tasks as needed. Snack served: CapriSun Fruit Punch and Baked Cheddar Goldfish. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Emergency preparedness supplies in order. Required documents posted. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on record review, a child with a diagnosed food allergy did not have a written allergy care plan. Evidence: Child#1 has a diagnosed food allergy, an Epipen Jr is kept on-site, the child's file and the medication authorization form were reviewed, neither contained a written allergy action plan. Child#5 has a diagnosed allergy, Benadryl is kept on-site, the child's file and the medication authorization form were reviewed, neither contained a written allergy action plan. Plan of Correction: I will give the form to parent and request that they have the children's doctors complete it.
Standard #: VENDSUB-000-031-B-3 Description: Based on record review of emergency procedures, the emergency preparedness plan did not contain all required information. Evidence: The emergency preparedness plan did not include lockdown procedures. Plan of Correction: I will have a lockdown plan added to our safety response plan.
