Comments:
Today's visit was a routine monitoring inspection. The inspection began at 8:55am and concluded at 3:00pm. There were twenty-seven children present with six staff members during the day of the inspection. During the inspection all five operating classrooms were observed, all three outdoor play areas were viewed, the end of breakfast was observed, lunch time was observed, five out of ten staff records were reviewed, five out of seventy-nine children's records were reviewed, emergency supplies were viewed for three buildings, the emergency preparedness plan was viewed, infant care was observed, and some policies and procedures were reviewed. Currently, the center is not using the classrooms in the lower portion of the building due to renovations. Discussion took place regarding updated background check requirements and reports of serious injury. Thank you for your time, please call me at (540)309-2051 if you have any questions.

Standard #: 22VAC40-185-50-A
Description: Based on the inspector's observations in the infant classroom, the center failed to treat children's records in a confidential manner. Evidence: The inspector observed a physician's note posted on a bulletin board as you enter the classroom. The note had the child's name and information regarding a recent doctor's visit. The child's information was not maintained in a confidential manner.

Plan of Correction: Staff will be reminded to place such notes in the children's records and information for children cannot be posted in a public manner.

Standard #: 22VAC40-185-70-A
Description: Based on the inspector's review of five out of five staff records, the center failed to ensure a portion of the records contained all of the required documentation. Evidence #1: The record for staff member #1 does not have the address for the individual listed as the staff member's emergency contact. Evidence #2: The record for staff member #3 does not have the address for the individual listed as the staff member's emergency contact. Evidence #3: The record for staff member #4 does not have two out two required references.

Plan of Correction: The administrator will obtain the information and add it to the staff member's records. Additional records will be audited for completeness.

Standard #: 22VAC40-185-570-B
Description: Based on the inspector's observations in the infant room, the center failed to ensure an infant was held while being fed a bottle. Evidence: The inspector observed an infant sitting in a molded chair and a staff was sitting next to them holding their bottle while they drank. the staff reported the child to be six months of age.

Plan of Correction: All staff who work in the infant classroom will be reminded to hold infants when they can't hold the bottles themselves.
