Comments:
A Subsidy Health and Safety inspection was conducted on 11/21/17 to review supplemental health and safety requirements. Today's visit was a routine monitoring inspection. The inspection began at 8:55am and concluded at 3:15pm. There were twenty-seven children present with six staff members during the day of the inspection. During the inspection all five operating classrooms were observed, all three outdoor play areas were viewed, the end of breakfast was observed, lunch time was observed, five out of ten staff records were reviewed, five out of seventy-nine children's records were reviewed, emergency supplies were viewed for three buildings, the emergency preparedness plan was viewed, infant care was observed, and some policies and procedures were reviewed. Currently, the center is not using the classrooms in the lower portion of the building due to renovations. Discussion took place regarding updated background check requirements and reports of serious injury. Thank you for your time, please call me at (540)309-2051 if you have any questions. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: VENDSUB-000-012-B
Description: Based on the inspector's review of five out of ten staff records, the center failed to ensure documentation for the completion of Virginia Preservice Training was not availbale for a staff member. Evidence: The record for staff member #2 with a hire date of 4/10/15 does not have documentation for the he completion of Virginia Preservice Training and the staff member has been employed for more than ninety days.

Plan of Correction: The staff member has completed the training and failed to provide the center her certificate. The staff member will provide the certificate by the end of the week.

Standard #: VENDSUB-000-031-B-1
Description: Based on the inspector's review of the emergency plan, the center failed to ensure the evacuation procedures have a plan to address reuniting children with parents or authorized person designated to pick up the child after an emergency evacuation. Evidence: The emergency evacuation plan does not have a reunification plan with regards to reuniting children with parents or individual authorized to pick up after an emergency evacuation.

Plan of Correction: A reunification plan will be added to the emergency preparedness plan.

Standard #: VENDSUB-000-031-B-2
Description: Based on the inspector's review of the emergency plan, the center failed to ensure the shelter in place procedures have a plan to address reuniting children with parents or authorized person designated to pick up the child after a shelter in place emergency. Evidence: The emergency evacuation plan does not have a reunification plan with regards to reuniting children with parent or individual authorized to pick up after a sheltering in place emergency.

Plan of Correction: A reunification plan will be added to the emergency plan.

Standard #: VENDSUB-000-031-B-3
Description: Based on the inspector's review of the emergency plan, the center failed to ensure the lockdown procedures have a plan to address reuniting children with parents or authorized person designated to pick up the child after a lockdown emergency. The lockdown procedure is also missing additional required components. Evidence #1: The emergency evacuation plan does not have a reunification plan with regards to reuniting children with parent or individual authorized to pick up after a lock down emergency. Evidence #2: The lockdown procedure does not include methods to secure the facility and designated lockdown locations.
