Technical Assistance:
An exit meeting was completed by the licensing inspector with the center's director to review the findings of today's inspection.

The licensing inspector conducted an unannounced renewal inspection on Tuesday, November 21, 2017 from approximately 1:50 pm to 5:30 pm to determine the center's compliance with licensing standards. During this inspection, the center's director was present and assisted the licensing inspector throughout this inspection. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 24 children. The staff to child ratios were deemed in compliance with licensing standards. The infants/toddlers were observed by the licensing inspector completing various activities to include sleeping in designated cribs, transitioning into snack time, sitting in bucket seats, and interacting with staff. The diapering area was observed by the inspector and determined to be in compliance. The licensing inspector observed the children in the Twos classroom napping in the designated areas. The Threes/Fours were combined in the classroom setting during napping in which they were supervised by staff. The children's lunch today consisted of turkey and cheese sandwich, sweet peas, apples sauce, and milk. The staff were observed by the licensing inspector providing age appropriate supervision and addressing the children's overall needs. The licensing inspector interacted with the staff and children when appropriate. The classrooms were equipped with age appropriate toys, and supplies. During this inspection, the center's classrooms, bathrooms, and overall facility was inspected. The licensing inspector observed the school aged children entering the facility. The required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, emergency supplies, menu, injury reports, fire and shelter-in-place drills, liability insurance policy, and children's allergies. Some of the licensing standards but not all were inspected today. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. There were five children?s records and five staff's records reviewed during this inspection. There are multiple staff with current MAT certification and Daily Health Observation training on site. The center's first aid kit and emergency supplies were inspected.

Standard #: 22VAC40-185-160-A
Description: Based on review of staff records, the center did not ensure to obtain documentation of the tuberculosis screening. Evidence: In Staff# 1's record (date of hire 10/25/17), there was not completed documentation of the staff's tuberculosis screening. The documentation of the screening shall be submitted no later than 21 days after employment.

Plan of Correction: The staff will obtain a copy of TB screening and will provide a copy to the director to be included in the staff record by November 22, 2017.

Standard #: 22VAC40-185-270-A
Description: Based on observation of inside areas at the center, the center did not ensure inside areas are maintained in a clean, safe, and operable condition.

Evidence: During this inspection, the inspector completed observations of the inside classroom areas to include school aged and twos, and there were multiple areas on the walls and window sills througout the classrooms where peeling paint was observed. These areas were accessible to children and presented a safety concern.

Plan of Correction: Maintenance team will touch up all areas with peeling paint throughout the building by Friday, December 8, 2017.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff record, the center did not ensure a central registry finding was received within 30 days of employment. Evidence: In Staff#3's record (date of hire 09/20/17), a completed central registry finding was not found within 30 days of employment. The inspector reviewed a document which indicated the document was resent on 10/24/2017 for corrective action, however, the document has not been received and no follow up has been completed by the center at this time.

Plan of Correction: An email has been sent by the center's management staff to inquire about the central registry finding status. The center will complete additional follow up by December 1, 2017.
