Licensing Inspector arrived at 9:05am and departed at 2:00pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 39 children present, ages Infant through five years in care with twelve staff members. Children were observed during circle time, playing in centers, and during outdoor play. Parents and guardians were also having a Thanksgiving Feast with the children and staff. Infants and Toddlers were observed feeding, during toileting and handwashing, during music and movement and playing freely. Five children's records and Five staff records were reviewed on this date. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on observation and review, the vendor did not ensure that there was a current list of all children's allergies, sensitivities, and dietary restrictions. Evidence: The list of children's allergies was not current, as a child's record was reviewed, and a documented food allergy was not included on the list.

Description: Based on record review, the vendor did not ensure that all required parent contact information was in each child's record. Evidence: There was no documentation of one or both parent's work addresses for all five children's records reviewed.

Plan of Correction: Work addresses will be included.

Standard #: VENDSUB-000-012-E
Description: Based on record review, in two of five staff records reviewed, the vendor did not ensure that all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval: Current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. The training shall include an in-person competency demonstration; and Current certification in first aid. Evidence: The following staff members did not have documenation of completing CPR and First Aid training within 30 days of the date of employment: Staff #1 (start date 9/17/17) Staff #3 (start date 9/18/17) Staff #5 (start date 9/11/17)

Plan of Correction: The class has been scheduled.

Standard #: VENDSUB-000-031-A
Description: Based on review, the vendor did not ensure that the center's written emergency preparedness plan addressed all required procedural components. Evidence: The emergency preparedness plan did not address lockdown procedures to include facility containment and all required components.
