An unannounced monitoring inspection was initiated today at 4:30pm. The physical plant, 5 staff files, 5 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 45 children under the supervision of 8 staff. During the inspection, children were observed playing outside, working on craft projects and homework, making cookies, and enjoying free play. Today, the children were served soft pretzel bites, mandarin oranges with the choice of white milk, chocolate milk or water. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Standard #: 22VAC40-191-40-D-3-D Description: Based on file review, 1 out of 5 staff records does not have all required background checks completed within the past three years. Evidence 1. Staff #1's most recent sworn disclosure statement (SDS) was completed on 8/7/14 2. Staff #1's most recent search of the central registry (SCR) was completed on 9/12/14. Plan of Correction: The SDS was printed off the website and completed during the inspection. The SCR will be submitted to be completed.
