Comments:
An unannounced inspection was conducted from 11:15 am to 12:25 pm. At the time of entrance, there were 19 children present with 3 staff present and 1 administrator. The inspector observed children participating in indoor play, lunch, toileting and hand washing procedures, and transitioning between activities. A sample size of 5 children's records and 5 staff records were reviewed, in addition to injury reports. Areas of noncompliance are identified in the violation notice and were discussed with the director at the conclusion of this inspection. Contact inspector at 757-354-7307 with any questions.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensue that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: On the outdoor playground there was: -Rusted and worn chain on the two swings closest to the playhouses -6 of 12 "S" hooks were open wider than the width of a dime

Plan of Correction: The chain will be replaced and the "S" hooks will be tightened.
