Comments:
The licensing inspector conducted an unannounced renewal inspection Wednesday, November 15, 2017 from approximately 10:00AM to 4:00PM and on Friday, November 17, 2017 from approximately 11:30AM to 1:20PM. The following ratios were observed with a total of 45 children present during the inspection: Early Head Start (1-2 years) - 6 children with 2 staff Early Head Start (2 years) - 7 children with 2 staff Head Start (Threes & Fours) - 16 children with 2 staff Preschool I (Infants ? 2 years) - 4 children with 1 staff Preschool II (Threes & Fours) - 12 children with 2 staff The children and staff were engaged in a variety of activities when the licensing inspector arrived. Children were observed being held and fed, creating turkeys from handprints, working puzzles, building with blocks, looking at books, playing in the gym, participating in circle time and eating lunch. Lunch consisted of pinto beans and rice chili with cheese, rolls, broccoli, pineapples and milk. Later children were observed resting at naptime. All areas of the center including classrooms, hallways, kitchen, playgrounds and bathrooms were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily sheets, Infant feeding schedules, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center?s first aid kit and emergency supplies were inspected. Medication is administered and medication and authorization forms were reviewed. Last documented health inspection: 09/15/2017 Last documented fire inspection: 11/15/17 During the inspection, five children's records and five staff records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. The renewal application was received prior to the expiration of the current license. Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure all areas were maintained in a clean, safe and operable condition.

Evidence:
During the inspection, the licensing inspector observed a broken electrical outlet located in the Early Head Start classroom. The electrical outlet plate did not cover the electrical receptacle, leaving a half inch gap at the top of the electrical receptacle where the electrical wires were located. The electrical outlet was located on the wall, approximately two feet from the floor.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements, the facility did not maintain the required depth of resilient surfacing in all fall zone areas for play equipment located on the playground. Evidence: During the inspection, the licensing inspector observed mulch at the end of a slide chute located on the playground. The height of the slide measured approximately four feet high. The licensing inspector took measurements of the depth of mulch at the end of the slide chute. The mulch measured approximately two inches in depth. Based on the height of the slide, six inches of depth in mulch was required in the fall zone surrounding the slide, including the end of the slide chute.

Description: Based on observation, the center did not ensure all staff washed hands after helping a child use the toilet or a diaper change.

Evidence:
During the inspection, the licensing inspector observed Staff #1 put on a pair of gloves and assist a child with toileting. The licensing inspector observed Staff #1 removed a soiled pull up off a child and then place the child on the toilet. After assisting the child with toileting, Staff #1 helped the child wash hands. After the child washed hands, the licensing inspector observed Staff #1 call over another child and remove the child's soiled pull up and place the child on the toilet. Staff #1 wore the same gloves while removing three pull ups and assisting three children to use the toilet. Staff #1 did not wash her hands after changing and assisting each child use the toilet.

Standard #: 22VAC40-191-60-C-1
Description: Based on the review of the five staff records and interviews, the facility did not obtain an original criminal history record report within 30 days of employment for one staff member, but the staff is still working. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 did not contain an original criminal history record report. The staff member began employment on 10/02/17. Criminal history record reports must be obtained within 30 days of hire. If the center does not have a criminal history record report for a staff within 30 days of employment, the center must deny continued employment until the report is received.

Description: Based on the review of the five staff records and interviews, the facility did not obtain a central registry finding within 30 days of employment for one staff member, but the staff is still working. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 did not contain a central registry finding. The staff member began employment on 10/02/17. Central registry findings must be obtained within 30 days of hire. If the center does not have a central registry finding for a staff within 30 days of employment, the center must deny continued employment until the finding is received.

Standard #: 63.2-1720.1-B-3
Description: Based on the review of the five staff records and interviews, the facility did not request a search of out-of-state founded complaints of child abuse for one staff member who has lived outside of Virginia within the last five years. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 10/02/17) indicated the staff member has lived outside of Virginia within the last five years. The record did not contain documentation of a request for founded complaints of child abuse from the states the staff members indicated they had lived in within the last five years. On 11/15/17, Staff #1 stated she had lived in another state in 2015. A member of management stated a request for the out of state child abuse registry search had not been requested as of 11/15/17. Staff #1 was observed working at the facility on 11/15/17.
