The center self-reported an incident that occurred on 11/2/2017 resulting in an accident that happened while a staff was transporting school-age children. After review of the documentation the center provided and review of the bus drivers staff file, a violation was cited in regard to safely transporting children. Please complete the columns for Plan of correction and Date to be Corrected for the violation cited on the violation notice and return a signed and dated copy the the licensing office within five (5) business days of receipt (12/7/2017). Description: Based on a self-reported incident, the center did not provide safe transportation from a school to the after school program. Evidence: On 11/2/17, the bus driver became distracted while driving 17 school-age children and ran off the road. The YMCA bus hit a sign along the road and ended up in the ditch.

Plan of Correction: The bus driver is no longer employed for the YMCA and the YMCA will provide additional supervision on the bus.
