Comments:
An unannounced monitoring inspection was conducted from 11:00am-1:00pm. Upon arrival there were 36 children in care with 7 staff present. A sample of 2 staff and 3 children's records were reviewed. The center did not have any medications and no children with food allergies. Children were observed during toileting, lunch and practicing for a holiday play.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review the center failed to ensure all required information was kept in each child's record. Evidence: Child #3 did not have a name, address or contact number for the child's father. The birth certificate listed the father and the father's name was listed on the "Not Authorized to Pick-Up List," however there were no court documents to support the request.

Plan of Correction: The director will contact the child's mother and find out additional information about the child's father and custody situation. The file will be updated accordingly.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the main hallway where the children hang their belongings there was an open tub of deodorizer gel on the shelf of the coat rack.

Plan of Correction: The gel was removed during the inspection.
