Technical Assistance:
Discussed new background check requirements, and training requirements for all teachers.

Comments:
An unannounced monitoring inspection was conducted today from 10:50am-1:00pm. There were 35 children directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play outdoors, engaging in imaginative play with rocks and sticks , reading books, singing songs in both Korean and English, and playing with legos. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A is missing documentation of a physical examination. (Child A, date of enrollment: 9/2017)

Plan of Correction: We will tell parents to get a documentation of a physical examination for the center.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas outside of the center were not maintain a safe condition. Evidence: 1) Two bucket swings were attached to the existing strap swings with zip chords. On one swing, the zip chord had broken and the bucket swing was only attached to the swing on one side.

Plan of Correction: The bucket swings were removed from the existing strap swings at the time of inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the 3 year old classroom directly next to the kitchen, a bleach and water solution was kept in an unlocked back cabinet.

Plan of Correction: The cleaner was removed and placed in a locked cabinet.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review of five staff records, the center did not have an original criminal history record report within 30 days of employment of a staff member. Evidence: Staff A does not have a complete criminal history record report within 30 days of employment. (Staff A, date of hire 10/1/2016)

Plan of Correction: The background check was previously sent and was sent back to us for more information but we never sent it out again. We will have the employee fill out a new central registry check immediately and send it out as soon as possible.
