Comments:
An unannounced monitoring inspection was conducted on 12/06/2017 from 10:45am - 1:10pm. At the time of entrance there were 27 children, ages 17 months - four years, in care with five staff. Children were observed during lunch and nap. Morning program time was observed in the older preschool classroom. Records were reviewed for two staff and five children. Administrative staff report no medication is being administered at this time. The violations were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Evidence: One outlet in the infant classroom lacked a protective cover. The outlet was above the leather sitting bench.

Plan of Correction: A protective cover was placed in the outlet during the inspection.

Standard #: 22VAC40-185-550-E
Description: Based on record review and observation, the center failed to maintain a record of the dates of the shelter-in-place practice drills. Evidence: 1. The facility emergency log did not have any shelter-in-place practice drills documented for 2017. 2. Two staff confirmed that two shelter drills had been conducted in 2017. a. The program director verified that the shelter drills had not been documented on the emergency log.

Plan of Correction: The drills will be documented on the emergency practice log for the school.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center failed to ensure that the record of children's minor injuries shall include: *Staff and parent signatures or two staff signatures; and *Time parent was notified of the injury. Evidence: 1. 3 of 7 injury reports reviewed lacked a second signature. 2. 5 of 7 injury reports reviewed lacked documentation of the time the parent was notified of the injury.

Plan of Correction: The Director will train the staff the proper way to complete the injury reports, which will include two signatures, the date and time that the parent was contacted about the injury.
